  • Proof of Federal Tax ID Number
  • Photo ID, DOB and SSN of all authorized users on account
  • Name of Business
  • Address (physical and mailing)
  • Phone Number
  • Fax Number
  • Email Address
  • Nature of Business

For the following types of businesses, you will also need:

Sole Proprietorship
Certificates of Assumed Name or DBA (Doing Business As) papers

Articles of Incorporation

Limited Liability Company
Articles of Organization
Operating Agreement (if not a single owner LLC)

Organization, Association and Non-Profits
Articles of Organization/Bylaws/Charter/Meeting Minutes

Certificate of Partnership
Partnership Agreement
