Certificate of Assumed Name - Doing Business As (DBA) Papers, if applicable
Proof of Federal ID number in the business name
ID (Drivers License) of all authorized users on account
Partnership
Certificate of Co-Partnership - Doing Business As (DBA) Papers, if applicable
